Because of insurance regulations, we are unable to provide referring physicians with a "referral form" for Audiology services. We request that you fax an order either on a prescription pad or letterhead to us. The following information needs to be on the order:
-Patient Name
-Patient Date of Birth
-Our name: Coastal Audiology
-"Evaluate and Treat, Hearing Loss"
-Diagnosis Code (typically 389.9 for unspecified hearing loss unless the diagnosis is known
-Date
You may fax it to the attention of our Receptionist, Deanna (pronounced Deen-a), at (912) 748-9495.
