Financial Policy

If you have been referred to Coastal Audiology by the Department of Veteran Affairs through VES, Logistics Health (LHI), or another compensation & pension third party, please click the links below to download and print the paperwork necessary for completing your claim with the VA. If you do not have access to a printer, please arrive no less than 15 minutes before your appointment time to complete the paperwork in the office. You may also complete the contact form to the right and someone will send you paperwork which can be completed online from a phone, tablet, or computer.
